Sara Johnson, MBA, PMP is the associate VP, population health and innovation for Duke Primary Care in Durham, North Carolina. In this role, she directs a range of adult and pediatric population health services, including annual wellness visits, chronic care management, behavioral health services, diabetes education and nutrition counseling, lactation services, and primary care pharmacy services. Under her leadership, Duke Primary Care Population Health launched the pediatric behavioral health program, behavioral health on call service for patients 18 and older, a hospital follow-up clinic, two new patient access clinics, and implemented standardized health related social needs screening across the network. Prior to joining Duke Primary Care in 2020, Sara worked for Duke Gastroenterology leading clinical operations. She earned her Bachelor of Science in Business Administration from the University of North Carolina at Chapel Hill and her Master of Business Administration from Elon University in Elon, NC.
